After the referral, parties will be contacted by our staff so that an intake interview can be booked.  Parties attend separate intake interviews.

 

What do custodial parties need to bring when attending the intake interview?

ChecklistAs the custodial party (the parent or guardian with custody of the children) you are required to bring the following to your intake interview:

  • Your full address and contact information
  • Names and telephone numbers of two of your friends or relatives so we can contact them in case of an emergency
  • Your child’s physician or pediatrician name and phone number
  • Your child’s health card number
  • All documents related to custody and access.  See Custody and Access Documentation List.
  • Your non-refundable application processing fee.  For details, see “Fees and payment options.”

As an access parent you are required to bring the following to your intake interview:

  • Your full address
